About The Position
Elara Caring is seeking a compassionate Licensed Vocational / Practical Nurse to provide home health care. The nurse follows individualized care plans and physician orders, monitors patient progress, communicates changes and safety concerns to the care team, and helps patients and caregivers learn self-care techniques. Working with the RN Case Manager, the nurse updates care plans as needs change, documents medication regimens, coordinates care with physicians, patients, and families, and provides proactive care intended to help prevent avoidable hospitalizations and emergency room visits. The role supports positive patient outcomes through compassionate, high-quality care.
Requirements
- Current state license as a Licensed Vocational / Practical Nurse.
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
- One year of experience in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
