About The Position

Elara Caring is seeking a Licensed Vocational / Practical Nurse for PRN home health care. The nurse provides compassionate, high-quality care in patients’ homes by following individualized care plans and physician orders, monitoring progress, coordinating with the care team, and supporting patients and caregivers with self-care. The role works with the RN Case Manager to update care plans as patient needs change and helps prevent avoidable hospitalizations and emergency room visits.

Requirements

• Current state license as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• At least 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
