About The Position

Elara Caring is seeking a compassionate Licensed Vocational / Practical Nurse to provide home health care. The nurse follows individualized care plans and physician orders, monitors patient progress, educates patients and caregivers, and coordinates with the RN Case Manager and other members of the care team. The role supports positive patient outcomes and helps prevent avoidable hospitalizations and emergency room visits. The position requires travel between patient homes and assigned work locations.

Requirements

• Current state license as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• At least 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
