About The Position

Lorian Health is hiring a skilled and compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to provide in-home nursing care under the supervision of a Registered Nurse (RN). The nurse will support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services. The role is based in San Diego South, with a Monday–Friday schedule and a weekend rotation. Pay is per visit, ranging from $35 to $45, commensurate with experience.

Requirements

• Current LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• At least one year of recent experience, within the last three years, in a home health or hospice setting.
• Current CPR/BLS certification.
• Valid driver’s license and a reliable, insured vehicle.
• Strong communication and clinical judgment, and ability to work independently.
• Ability to lift or transfer patients and perform physical caregiving tasks.
• Ability to pass required physical exams and annual PPD testing.

Responsibilities

• Provide nursing care according to the RN-developed Plan of Care.
• Monitor and document changes in patient condition, and report them to the RN or physician as needed.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within the LPN/LVN scope of practice.
• Maintain accurate EMR documentation.
• Participate in case conferences and coordinate care with the home health team.
• Follow infection-control, HIPAA, and company policies.
