About The Position
HQ Home Care - Certified Health Quest Home Care is seeking a Home Health Aide to assist patients with personal care activities, exercises, ambulation, and meal preparation. The role involves monitoring patient condition, taking vital signs, and providing medication prompts. This position requires significant manual skills, motor coordination, and dexterity, and involves potential occupational risks and exposure to various environmental factors.
Requirements
- Active NYS Home Health Aide Certification
- Driver's License
Responsibilities
- Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
- Assists the client with exercises, ambulation, and transfer activities, including the use of assistive devices such as canes, walkers, crutches, and lifts.
- Independently assists the patient with meal preparation/cooking and/or feeding, with knowledge of special diets and sanitary kitchen conditions.
- Assists the patient with medication prompts to remind the patient to self-administer his/her medications.
- Identifies changes in patient condition that require notification to the patient’s registered case manager.
- Takes vital signs to include temperature, pulse, heart rate, respiratory rate.
