About The Position
LifeCircles PACE is seeking a dedicated and compassionate Home Health Aide (HHA) to provide in-home care and support to older adults, helping them remain safely in their homes while maintaining their dignity and independence. The role is in the Home Care department of LifeCircles PACE in Holland, Michigan, and follows a 40-hour-per-week schedule, Monday through Friday, with holiday rotation, from 8:00 a.m. to 4:30 p.m. The aide provides personal care in accordance with each participant’s Life Plan, supports household and mealtime needs, assists with mobility and safe transfers, measures and records health observations, and reports changes in participants’ conditions to the interdisciplinary team and leadership. The position also requires respectful, professional, and confidential care. The listed duties are a summary and are not necessarily the only duties required.
Requirements
- Certified Nurse Aide (CNA) credential meeting state requirements.
- Reliable transportation and a valid driver’s license.
- High school diploma or GED.
- Excellent communication and observation skills.

Responsibilities
- Assist participants with activities of daily living, including bathing, grooming, dressing, and toileting, in accordance with their Life Plan.
- Support housekeeping tasks such as laundry, sweeping, and grocery shopping.
- Assist participants with meals and hydration.
- Support mobility, including turning, positioning, and safe transfers.
- Measure and record vital signs, weight, and intake and output.
- Keep participants’ rooms tidy and well-stocked with essentials.
- Respond to participants with kindness and compassion.
- Report changes in participants’ conditions promptly to the interdisciplinary team and leadership.
- Maintain confidentiality and demonstrate professionalism while providing care.
