The stroke coordinator is responsible for coordinating and implementing all stroke services and activities for the assigned community. He/she functions as a liaison between the LIPs, nursing staff, allied healthcare professionals, patients and their families; coordinates and facilitates the care for the stroke patient throughout the continuum; works closely with LIPs and nursing to ensure clinical practice guidelines are followed. The stroke coordinator participates in program development as it relates to the hospital and community. This includes, but is not limited to staff education, community education, and marketing. He/she is responsible, in coordination with the stroke program medical director, for planning, developing and implementing stroke center initiatives. Will maintain quality standards for responsive service including data management and performance improvement for key quality indicators. He/she will take a leadership role in development and implementation of outcomes evaluation as guided by DNV standards and evidence-based guidelines.
