Requirements

• Master’s degree in social work.
• Valid driver’s license issued in the United States; driving eligibility is determined under FDL-approved guidelines.

• Travel is required.

Responsibilities

• Assist clients in navigating inpatient medical stays and support culturally appropriate care in inpatient settings.
• Assist clients with obtaining Durable Medical Equipment (DME).
• Accept and record referrals from outside providers and ensure their completion; conduct client intake interviews.
• Develop plans of care with clients and families when appropriate, offer in-network options, identify alternative resources, and provide follow-up services.
• Participate in inter-professional, team-based primary care casework analysis and planning; exchange relevant information with team members and provide feedback to referring providers when appropriate.
• Coordinate placement in rehabilitation programs and substance-abuse treatment facilities when necessary.
• Assist with hospital discharge planning using shared decision-making to support safe transitions to home, rehabilitation, assisted living, or long-term care.
• Help patients obtain insurance coverage for long-term care and understand policy coverage; refer patients to FDL HSD Patient Advocates or care coordinators for assistance obtaining other insurance coverage.
• Coordinate patient care conferences, including scheduling with providers, outside agencies, family members, or a power of attorney as applicable.
• Assist with forms requiring a provider’s signature, including life-sustaining treatment, disability, medical necessity, school and childcare health, insurance, workers’ compensation, FMLA, DME, and prescription-related forms.
• Participate in patient care conferences and advocate for patients by identifying unmet needs and helping develop strategies to address them.
• Coordinate care and shelter services for people facing homelessness, sexual violence, or other significant challenges; assist with reporting abuse or neglect to appropriate authorities.
• Coordinate care with inpatient facilities not associated with FDL, FDL or county social services, and FDL Home Care or other home care providers.
• Assist patients with advance care directives upon request and with Medical Assistance restricted-patient requirements or changes.
• Connect veterans with the FDL or county Veteran Service Office and educate patients and families about alternative community resources.
• Assist medical department leadership in identifying unmet client needs and ways to address them.
• Collaborate across HSD departments, including medical, community health, social services, dental, behavioral health, and substance use, and support collaboration across the healthcare continuum.
• Maintain awareness of financial and medical resources in the service area and stay informed about county, state, federal, educational, vocational, and therapeutic programs for Indian people.
• Document client services in the electronic health record.
• Attend mandatory department training, maintain confidentiality of financial and employee, band member, and client records, and maintain a clean and organized work area.
• Respond promptly and professionally to requests, complaints, and problems; represent the Fond du Lac Band and department positively and respectfully in the community.
• Maintain proper attendance and punctuality, wear appropriate work attire, and remain flexible in assuming other responsibilities assigned by management.

Benefits

• $5,000 bonus with a two-year commitment.
