The Social Worker in the Transplant Program completes in-depth assessments of patients to evaluate candidacy for solid organ transplant and provides counseling, support, and community resource referrals to patients and families preparing for transplant. The role follows patients before and after transplant to help resolve psychosocial issues and support lifestyle changes that improve post-transplant success. The Social Worker collaborates with the multidisciplinary transplant team to assess inpatients and develop appropriate discharge plans, and provides assessment and intervention planning for patients experiencing substance abuse or mental health issues. The role advocates for patients who may be victims of child abuse, domestic violence, or adult/elder abuse; supports education and supervision of MSW interns; participates in community outreach and patient education; and assists the manager with policy development, problem solving, program development, and staff training. The position completes transplant evaluations, crisis interventions, short-term counseling and treatment, community referrals, documentation, and reporting. It also evaluates potential living donors and provides appropriate post-surgery follow-up to donors and their families, while collaborating with the transplant team on regulatory issues. This is a non-management role reporting to a supervisor, manager, director, or executive.
