Social Worker (Hospice) - Full Time - Rockport
About The Position
The Social Worker provides social services to patients and their families, including treatment planning and consultation with other staff members. The role involves assessing patients’ and families’ emotional and social factors to estimate their capacity and potential. The Social Worker also assists with discharge planning when agency care is no longer appropriate by submitting a written summary of services and an assessment of the patient’s current status for the patient’s chart. The position participates in discharge planning and in-service programs related to patient needs.
Requirements
- Licensed Social Worker in the State of Texas.
- At least 1 year of experience with a social work license.
- Effective written and verbal communication skills.
- Clean background, drug screen, and driving record.
