About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout the DMV region, including Washington, DC, Prince George's County, Montgomery County, Fairfax County, Loudoun County, Prince William County, Falls Church, Alexandria, and Arlington. This PRN opportunity offers flexibility, autonomy, and the ability to provide exceptional one-on-one patient care in patients' homes. As a Home Health Registered Nurse, you will build meaningful relationships, provide education and support, coordinate care, and help patients manage their health while maintaining their independence. The role focuses on improving outcomes, preventing hospitalizations, and enhancing a patient's quality of life through meaningful nursing care.
Requirements
- Active RN License Required in DC, Maryland, Virginia, or Compact State License with eligibility to practice in assigned service areas
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Organizational Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- Wound Care, Case Management, or Acute Care Experience a Plus
Responsibilities
- Perform comprehensive patient assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services and patient education
- Monitor patient progress and communicate changes to physicians and care teams
- Coordinate care with therapists, social workers, and other healthcare professionals
- Complete timely and accurate clinical documentation
- Support patients and families throughout their healthcare journey
