About The Position

Requirements

• Current and unrestricted occupational therapist licensure.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Nice To Haves

• At least six months of occupational therapy experience.
• Home Health experience.

Responsibilities

• Assess and screen patients' daily living and work-related skills, and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to help physicians evaluate patients' functional levels.
• Confer with physicians and other healthcare team members and participate in developing and revising treatment plans.
• Provide physician-ordered therapeutic treatment and instruction to improve or restore strength, coordination, range of motion, and function, or teach compensatory measures.
• Review and expand retraining programs in line with each patient's behavioral goals and the rehabilitation program.
• Consult with vocational team members to develop and implement plans consistent with patients' needs and capabilities.
• Report patient status and progress to physicians, rehabilitation staff, the Clinical Manager, patients, and family members.
• Participate in care coordination and discharge planning, and act as a resource to other healthcare personnel.
• Design community reintegration activities and coordinate plans with the interdisciplinary team.
• Recommend or design adaptive equipment to improve patients' home or work environments or participation in rehabilitation and community activities.
• Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in educational meetings.
• Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
• Document care observations, interventions, and evaluations accurately and thoroughly on the day services are provided.
• Accurately document and implement interim verbal orders received from physicians.
• Submit evaluations, treatment plans, and discharge summaries to the supervisor and care management staff.
