About The Position

Requirements

• Associate degree in Nursing from an accredited college or university is minimally required.
• Bachelor’s degree in Nursing from an accredited college or university is required to facilitate complex disease care management and advanced patient needs.
• Licensed Practical Nurse (LPN) or Registered Nurse (R.N.) with a valid license in the state of Illinois
• Current CPR Certification

Nice To Haves

• A minimum of two (2) – three (3) years’ experience as a qualified Case Manager preferred.
• Significant demonstrable experience of working in a community-based environment.
• Significant demonstrable experience of working with highly complex packages of care in a health and/or social care environment.
• Bilingual ability preferred; may or may not be required based on individual department needs.

Responsibilities

• Managing the ongoing care management for an assigned population case load (Population Health Management)
• Performing Comprehensive Risk Assessments and developing individualized Patient Care Plans for all assigned cases
• Managing and coordinating patient access to care, including overseeing the efforts of collaborating Care Coordinators, Referral staff and others
• Addressing all relevant Transitions of Care management when patients obtain hospital-based care
• Collaborative participation in the Patient Care Teams
• Population Health Management
• Development & Management of Patient Care Plans
• Management and coordination of care
• Transitions of Care and Self Management
