About The Position

Sutter West Bay Medical Group is seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our multidisciplinary Neuromuscular Disease & ALS Center of Excellence. This unique opportunity offers the ability to work alongside leading neuromuscular specialists while serving as a key clinical partner for patients and families navigating complex neurological conditions. Requirements

  • Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program
  • Current California NP or PA licensure (or ability to obtain)
  • National certification
  • DEA registration (or eligibility)
  • Minimum three years of advanced clinical experience
  • Strong clinical judgment and patient-centered care philosophy
  • Excellent communication and care coordination skills
  • Ability to build trusted relationships with patients, families, and interdisciplinary teams
  • Passion for caring for individuals with complex neurological disorders
  • Interest in research, innovation, and continuous quality improvement
  • Ability to work independently while thriving in a collaborative team environment

Responsibilities

  • Conduct comprehensive patient evaluations, including symptom management, medication review, functional assessments, and ongoing monitoring
  • Develop individualized treatment plans in collaboration with supervising neurologists and interdisciplinary team members
  • Coordinate care across multiple specialties and services to ensure seamless patient experiences
  • Support advanced care planning and goals-of-care discussions
  • Facilitate access to specialty therapies, medications, assistive technologies, and supportive resources
  • Serve as a trusted resource for patients and caregivers throughout the continuum of care
