About The Position

Requirements

• High school diploma or equivalent.
• Two or more years of experience in sales and/or marketing.
• Must have and maintain a valid driver's license.

Responsibilities

• Collaborate with hospital discharge planners, social workers, case managers, physicians, management, and education staff to generate referral sources.
• Develop and maintain strong relationships with key referral sources.
• Manage leads from referral sources and support an efficient admissions process for clients.
• Increase the number of referral sources within facilities, including physicians, social workers, and discharge planners.
• Coordinate with the clinical assessment team on admission referrals based on the services provided by the facility.
• Educate patients, families, community members, and other referral sources about the facility’s programs, services, and amenities.
• Coordinate completion of the admission process and ensure appropriate resources are obtained.
• Travel frequently within the community to schedule meetings and develop contacts and referral sources.
• Use analytic tools such as Salesforce to plan and evaluate activity and optimize results.
• Complete on-site patient assessments to coordinate admissions.
• Assist with transportation as needed.
• Perform other functions and tasks as assigned.
