About The Position

Life Center of Galax is seeking a Mobile Assessor to expand lines of service by developing and retaining referrals from physicians and other key referral sources. The role focuses on building referral relationships, coordinating client admissions, and educating patients, families, community members, and referral sources about the facility’s programs and services. The position is not remote; the posting says the person should live in the Hampton Roads, Williamsburg, or Tidewater area.

Requirements

High school diploma or equivalent.
Two or more years of experience in sales and/or marketing.
Must have and maintain a valid driver's license.

Bachelor's degree in Marketing or a related health or social services field.

Responsibilities

Collaborate with hospital discharge planners, social workers, case managers, physicians, management, and education staff to generate referral sources.
Develop and maintain strong relationships with key referral sources.
Manage leads from referral sources and support an efficient admissions process for clients.
Increase the number of referral sources within facilities, including physicians, social workers, and discharge planners.
Coordinate with the clinical assessment team on admission referrals based on the services provided by the facility.
Educate patients, families, community members, and other referral sources about the facility’s programs, services, and amenities.
Coordinate completion of the admission process and ensure appropriate resources are obtained.
Travel frequently within the community to schedule meetings and develop contacts and referral sources.
Use analytic tools such as Salesforce to plan and evaluate activity and optimize results.
Complete on-site patient assessments to coordinate admissions.
Assist with transportation as needed.
Perform other functions and tasks as assigned.
