About The Position

Requirements

• At least 1 year of clinical LPN/LVN experience.
• Current LPN/LVN license specific to the state(s) where assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.

Nice To Haves

• At least 1 year of hospice experience or experience caring for chronically or terminally ill patients.
• Knowledge of the physical, psychosocial, and spiritual needs of hospice patients and their caregivers.

Responsibilities

• Verify the plan of care with the RN before each visit.
• Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient’s condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by the physician, such as Foley catheter changes.
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation accurately and on time.
