Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services. The CCBHC Veterans and Criminal Justice Liaison (“Liaison”) is responsible for: (A) ensuring that veterans and certain U.S. Armed Forces members have access to intensive, community-based treatment within the Certified Community Behavioral Health Clinic (CCBHC) as well as continuity of care; (B) supporting clients to minimize contact with law enforcement and the criminal justice system while promoting individual and public safety; and (C) ensuring continuity of care for CCBHC clients involved in the criminal justice system (i.e. through incarceration, parole, probation, juvenile detention, diversion program, arrest, Marion County specialty courts, Marion County Re-Entry Coalition, or any other facet or extension of the criminal justice system). The Liaison provides case management and linkage to social determinants of health, primary care, and other needed resources. The Liaison is responsible for developing and maintaining relationships with the systems that impact veterans and/or those involved in the criminal justice system. The Liaison maintains close collaboration with the CCBHC outpatient treatment teams and continues to actively work with CCBHC clients in the community until the primary CCBHC treatment team can fully engage the client. The Liaison additionally has the important role of tracking and reporting their work with CCBHC clients consistent with CCBHC quality metrics.
