About The Position

The Assertive Community Treatment (ACT) program delivers comprehensive and effective services to individuals diagnosed with severe mental illness whose needs have not been well met in traditional outpatient settings. ACT is an evidence-based practice that provides psychiatric treatment, case management, and support services through a multi-disciplinary mental health treatment team. The Wellness Self-Management Specialist collaborates with other ACT treatment team members to engage clients in treatment, identify person-centered goals, and empower clients to develop essential life skills. This role involves providing services to individuals and in group settings. It is a field-based position that requires community outreach visits with clients in their residences, shelters, and other locations to ensure their continued engagement in treatment.
