Requirements

• Graduated from an accredited nursing program within the past 12 months.
• Obtain nursing license and registration within the timeframe set by state regulations or within 90 days of hire, whichever occurs first.
• Hold a current appropriate state Temporary Permit.
• Meet the practice requirements of the state where employed.
• Maintain any required credentials or certifications in active and valid status.
• Be capable of handling demanding situations, multitasking, and adapting to changing needs.
• Maintain regular and punctual attendance.
• Ability to perform the physical demands of direct patient care, including heavy lifting and moving of patients, assisting with ambulation, prolonged standing, bending, and occasionally moving equipment or lifting solutions as described.
• Take the Ishihara color blindness test as a condition of employment; failing the test does not preclude employment, and reasonable accommodation may be considered.

Responsibilities

• Direct and monitor patient-care staff to ensure safe, effective hemodialysis care in compliance with facility procedures and applicable regulations.
• Delegate tasks to direct patient-care staff, including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.
• Assess daily patient-care needs and develop appropriate patient-care assignments.
• Monitor staff techniques and adherence to facility policies and procedures; participate in staff training, orientation, and required meetings.
• Educate patients and families about ESRD, renal disease, vascular access, dialysis therapy, diet and fluid management, medications, laboratory values, treatment adherence, transplant awareness, and treatment modalities.
• Provide and monitor hemodialysis care, implement prescribed treatment parameters, assess patient responses, and adjust care as indicated by the prescribing physician.
• Communicate patient issues and concerns to the Team Leader or physician and collaborate with physicians and other healthcare team members to coordinate care.
• Complete initial and annual nursing assessments and ongoing evaluation and documentation of patient-care needs.
• Participate in pre-evaluation, treatment initiation and monitoring, termination, access homeostasis, and post-evaluation for hemodialysis patients.
• Address changes in adequacy status and troubleshoot access-flow issues identified through OLC/AMP monitoring.
• Provide, supervise when applicable, and monitor hemodialysis access care.
• Implement and document prescribed interdialytic transfusions and notify the physician and blood supplier of adverse reactions as appropriate.
• Review, transcribe, and enter physician laboratory orders; prepare requisitions; ensure correct specimen collection, processing, packaging, and delivery; follow up on missed or insufficient specimens; and report abnormal or alert results.
• Enforce company policies and applicable state and federal regulations, and maintain safe, efficient, and effective shift operations.
• Act as a resource for staff and meet routinely with the Clinical Manager about personnel and patient-care matters.
• Assist patients with prescription refills and coordinate transportation when necessary.
• Maintain accurate patient documentation and medical records, including orders, laboratory results, vital signs, treatment parameters, and patient status; verify Patient Care Technician documentation.
• Maintain the facility drug list, controlled-substance records, and adequate emergency equipment; administer prescribed or approved-algorithm medications and document required assessments and justifications.
• Maintain competency in emergency procedures and initiate CPR and emergency measures in the event of cardiac or respiratory arrest.
• Ensure appropriate vaccinations, immunizations, and annual tuberculosis testing.
• Maintain a clean, safe, and sanitary dialysis treatment area; ensure machines, chairs, and stations are clean and properly positioned; promptly address blood spills according to policy.
• Complete opening and closing procedures and ensure staff and patients have safely left the premises.
• Initiate, document, and complete monthly CQI activities and reports, nurses’ progress notes, patient-care plans, and other required records and programs.
• Review transplant status and follow established procedures; complete annual nursing history and physical assessments and annual standing-order reviews with physicians.
• Maintain required credentials and certifications in active, valid status and promptly notify a supervisor of any change in credential status.
• Maintain regular and punctual attendance, remain flexible in a demanding and changing environment, and perform other related duties as assigned.
