About The Position

This role conducts clinical assessments and reassessments with referred customers, reviews medical and other health-related documents and conditions using standard procedures, and reviews any prior documentation, as applicable. The position also involves conducting bio psychosocial assessments using approved questionnaires and techniques, and performing the Social Security Administration sequential evaluation process for determining disabilities. Additionally, the role includes conducting mental health evaluations for customers identified as in need of evaluation as a result of the clinical assessment process, identifying customer functional limitations, and making a functional capacity outcome determination. The Qualified Health Pro may request or conduct additional medical or other assessments to support making a functional capacity outcome determination. They will coordinate Review Physician and Evaluating Clinician activities as required, work with the Engagement Specialist to ensure customer outreach for follow-up activities is conducted, and compile clinical assessment forms and documentation, preparing clinical assessment reports as required by HRA. The role involves signing off on and gaining customer signatures on clinical assessments when completed, and working collaboratively with service track staff to interpret clinical assessment findings or documents that affect service planning and delivery. The position utilizes motivational interviewing techniques to elicit the most complete information from customers during the assessment process, participates in team meetings to evaluate and improve clinical assessment processes, and engages in any required training related to the position and its responsibilities. The Qualified Health Pro must understand and adhere to HRA policies and procedures and RWS standard operating procedures related to clinical assessments.

Requirements

  • Must be a licensed or certified health professional: Licensed Clinical Social Workers (LCSW), Certified Rehabilitation Counselors (CRC), Licensed Mental Health Counselor (LMHC) and/or other credentialed professionals as may be approved by HRA
  • Four years clinical assessment experience that includes review of documents, bio psychosocial assessments, information interviewing, and SSA assessment of disabilities
  • Experience in a fast-paced, team environment that is performance driven
  • Exceptional customer service and engagement skills; training or experience in motivational interviewing and care management techniques
  • Excellent organizational skills and strong attention to detail
  • Experience with publicly-funded programs; TANF experience preferable
  • Knowledge of laws and regulatory requirements related to program responsibilities
  • Ability to use various electronic systems to carry out duties and responsibilities

Responsibilities

  • Conducts clinical assessments and reassessments with referred customers
  • Reviews medical and other health-related documents and conditions using standard procedures
  • Reviews any prior documentation, as applicable

  • Conducts the Social Security Administration sequential evaluation process for determining disabilities
  • Conducts mental health evaluations for customers identified as in need of evaluation as a result of the clinical assessment process
  • Identifies customer functional limitations and makes a functional capacity outcome determination
  • May request or conduct additional medical or other assessment to support making a functional capacity outcome determination
  • Coordinates Review Physician activities as required when the Qualified Health Professional is unable to make the functional capacity outcome determination of if the customers functional capacity outcome is fully employable or vocational rehabilitation
  • Coordinates Evaluating Clinician activities as needed to conduct further medical, mental health, or substance abuse assessment as may be needed to determine the customer’s functional capacity outcome
  • Works with the Engagement Specialist to ensure customer outreach related to any needed follow-up activities is conducted
  • Compiles clinical assessment forms and documentation and prepares clinical assessment reports as required by HRA
  • Signs off on and gains customer signatures on clinical assessments when completed
  • Works collaboratively with service track staff to interpret any clinical assessment findings or documents that affect service planning and delivery
  • Uses motivational interviewing techniques to elicit most complete information from customers during the assessment process
  • Participates in team meetings to evaluate and improve clinical assessment processes, including providing input to the design of any clinical assessment forms
  • Participates in any required training related to the position and its responsibilities
  • Understands and adheres to HRA policies and procedures and RWS standard operating procedures related to clinical assessments
