About The Position
We are currently seeking a Full-Time Case Manager (RN) to join our outstanding healthcare team! This is a Hybrid position meaning you will be traveling to Tacoma, Seattle and Puyallup to help support but your home base will be here at Montesano Health and Rehab. We are passionate about providing high-quality care and fostering a compassionate, family-oriented environment. We believe in building a positive and collaborative culture that prioritizes trust, communication, and person-centered care. As a Full-Time Hybrid Case Manager (RN), you will oversee and manage the delivery of care for residents from pre-admission through post-admission. You will play a pivotal role in ensuring residents and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.
Requirements
- Successfully graduated from an approved and certified RN program
- Current and unencumbered license to practice as an RN in the state
- Ability to make quick judgments and work independently
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Nice To Haves
- 2 years of experience as a RN
- Active patient or bedside care experience within the past three years
Responsibilities
- Managing care for residents throughout the care continuum, including pre-admission, admission, and post-admission stages.
- Coordinating services to ensure that residents' needs are met in a timely and effective manner.
- Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
- Monitoring the progress of residents and ensuring appropriate interventions are implemented when necessary.
- Facilitating smooth transitions of care, ensuring all services and resources are in place for each resident.
- Advocating for residents and families, ensuring their needs are met, and providing education as needed.
- Supporting/traveling to other buildings.
