Georgia Pines Community Service Board is a public, not-for-profit organization with an annual budget of $25M and a diverse workforce of 300 direct-care and support staff. We provide services to 6 rural counties with several locations. We strive to provide the highest quality, evidence-based services in the least restrictive settings. Georgia Pines Mental Health Lifted Program is currently seeking someone who has the passion, commitment, talent, and ability to work collaboratively with their peers to continue to advance the mission and vision of the organization. The selected applicant will support individuals impacted by substance use, mental health challenges, and incarceration. The case manager plays a critical role in the re-entry process by coordinating care, connecting clients to community resources, assisting with discharge planning, and promoting continuity of services following release from jail. The case manager needs strong engagement, organization, advocacy, documentation, and data tracking skills. The program serves and works within 4 county jails to include Decatur, Grady, Mitchell, and Thomas. This position is an integral part of the agency's Lifted program and reports directly to the Team Lead with appropriate latitude for utilizing seasoned judgment to accomplish his/her responsibilities.
