About The Position
Home Care by Black Stone, a part of LHC Group and Optum, is seeking a Home and Community Based Services Aide. This role involves providing support and assistance with personal hygiene and household functions to individuals, enabling them to remain in their own homes. The company is dedicated to helping people feel their best and fostering meaningful connections within its team and communities.
Requirements
- Current driver’s license, vehicle insurance, and reliable transportation or access to public transit
Nice To Haves
- 6 months+ of home care experience
- Current CPR certification or ability to complete within 90 days of hire
- Ability to work flexible hours
Responsibilities
- Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
- Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
- Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
- Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
