About The Position
Home Care by Black Stone, a part of LHC Group and Optum, is seeking a Home and Community Based Services Aide. This role is dedicated to helping people feel their best by creating meaningful connections with patients, their families, and communities. The aide will provide support and assistance with personal hygiene and household functions, enabling individuals to remain in their own homes.
Requirements
- Current driver’s license, vehicle insurance, and reliable transportation or access to public transit
Nice To Haves
- 6 months+ of home care experience
- Current CPR certification or ability to complete within 90 days of hire
- Ability to work flexible hours
Responsibilities
- Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
- Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
- Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
- Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
