About The Position
The Care Navigator Case Manager leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners. HCRS is a team of committed providers and business leaders passionate about transforming the lives of those at risk. We strive for excellence and have achieved significant and measurable improvement in clinical outcomes for the more than 4,000 clients we serve.
Requirements
- An Associate’s Degree and two years experience and/or Bachelor’s Degree.
- The ability to establish and maintain supportive relationships with children, youth and families.
- Knowledge on how to provide holistic and therapeutic case management services.
- Experience working in the mental health field.
Responsibilities
- Lead service planning and care coordination navigation for children and families receiving mental health services.
- Manage referrals.
- Provide primary care screening and monitoring.
- Serve as a primary liaison with community partners.
