About The Position

Join a high-growth, mobile healthcare team serving seniors across personal homes, board and cares, assisted living, and skilled nursing facilities. This Float Advanced Practice Provider (NP/PA) role supports a large regional footprint by providing coverage for PTO, turnover, and new building launches, ensuring continuity of high-quality, relationship-based care. It is a high-visibility position ideal for experienced clinicians who are adaptable, operationally strong, and interested in future leadership opportunities. The role offers regional float coverage across home-based care and senior living communities, leadership-track potential (future pod ownership or regional growth roles), autonomy with strong clinical and operational support, and a continuum-based care model integrating primary care, geriatrics, psych, and palliative care. Compensation includes a stable base salary plus productivity.

Requirements

  • Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license
  • Comfortable practicing independently in mobile/community settings
  • Strong clinical judgment and panel management skills
  • Adaptable, organized, and growth-oriented
  • Valid driver’s license and ability to travel within region

Nice To Haves

  • 2+ years experience in geriatrics, primary care, home health, SNF, or senior living preferred

Responsibilities

  • Provide mobile primary care to seniors in homes, boarding cares, ALF, SNF, and hospice settings
  • Step into established panels to ensure seamless patient continuity
  • Support new market expansion and building launches
  • Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)
  • Deliver dementia-informed, relationship-based care
  • Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)
  • Maintain strong documentation, communication, and quality standards
