About The Position

Scotland Memorial Hospital - Oncology Clinic is seeking a full-time Social Worker to provide quality social work assessment and intervention to meet the psychosocial needs of patients of SCTC departments and their families and significant others. The role involves enabling patients and families to understand and utilize services, enhancing their coping mechanisms, and working with case managers to ensure timely discharge planning. The Social Worker will also make necessary referrals, plan support groups and educational activities, provide counseling, and interact with physicians and midlevel providers to minimize treatment delays and ensure cost-effective care. Documentation of all social work activities is required, along with age-specific competency for adolescent, adult, middle-aged, and geriatric patients. The position also involves collaborating with financial navigators, nurses, and pharmacists to assist with patient assistance programs and grants, and acting as a liaison between the Scotland Cancer Treatment Center and community agencies.

Requirements

  • Minimum of one year of professional experience in a hospital or health-related setting.
  • Expertise with data management tools.
  • Adherence to National Association of Social Workers Code of Ethics.
  • Knowledge of human growth and development through the life span of the patient and ability to meet the needs of and provide care for patients in the following age categories: Adolescence 13 - 17 yrs; Young Adult/Adult 18 – 64 yrs; Adult/Middle-Aged 65 - 79 yrs; Geriatric 80+ yrs.

Responsibilities

  • Provide quality social work assessment and intervention to meet the psychosocial needs of patients of SCTC departments and their families and significant others.
  • Enable the patient and family to understand and utilize to the fullest extent services and education provided by the cancer center and/or other agencies and resources and to be knowledgeable regarding cancer and the specific disease process.
  • Enhance the overall functioning and coping of the patient and his/her family within the community through a counseling relationship.
  • Work with the inpatient Case Manager/ Discharge Planner to assure appropriate and timely completion of the discharge planning process for all patients who have discharge planning needs.
  • Makes any necessary referrals for the patient and his/her family in collaboration with other interdisciplinary team members as needed, with appropriate follow-up.
  • Plan and implement any necessary support groups, educational activities, and celebrations for cancer patients and their families.
  • Provide social adjustment, supportive, and crisis intervention counseling for all cancer patients and their families, as needed.
  • Interact daily with assigned Physicians and Midlevel providers in order to identify potential barriers and suggest interventions to minimize treatment delays, assure patients are treated at the most appropriate level of service, and assure cost effective treatment.
  • Document all social work activities.
  • Other duties as assigned by the SCTC Manager or SCTC Director.
  • Staff members will have knowledge of human growth and development through the life span of the patient and have the ability to meet the needs of and provide the care of these patients. The age categories to be included are: Adolescence 13 - 17 yrs; Young Adult/Adult 18 – 64 yrs; Adult/Middle-Aged 65 - 79 yrs; Geriatric 80+ yrs
  • Works closely with financial navigator, oral chemotherapy nurse, and pharmacist to assist with patient assistance programs as needed.
  • Assist with locating and applying for grants from outside sources to fund patient needs to fund identified patient needs.
  • Act as a liaison between the Scotland Cancer Treatment Center and community agencies (ie Hospice of Scotland County, DSS, etc).
