About The Position

Sage Group HealthCare Staffing is seeking Home Health Registered Nurses for multiple travel/contract positions in the Seattle region, WA. We are accepting both local and traveler candidates. This full-time contract position is for 13 weeks, with a guaranteed 40 hours per week, Monday through Friday, with no weekends or on-call duties. The role involves providing and coordinating patient care in their homes, developing and implementing nursing care plans, supporting patient recovery, educating patients and families, and performing case management. The nurse will ensure physician's orders are followed, assess patient status, notify physicians of changes, and complete charting within 24 hours. Territories to cover include King County (at and around West Seattle/Downtown Seattle).

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursing experience: 1 year within the last 3 years - Required
  • OASIS experience - Required
  • Home Care Home Base (HCHB) charting experience
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State of the state applying or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned patients in the comfort of their homes.
  • Develop and implement nursing care plans for all types of visits including Start of Care and admissions as assigned.
  • Support patient recovery and ability to stay at home.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Case management and provide clinical oversight to interdisciplinary teams.
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
