About The Position
Family Pillars Home Care is expanding its clinical team and is seeking compassionate, reliable Registered Nurses to complete Start of Care (SOC), Resumption of Care (ROC), Re-certifications, and Skilled Nursing Visits in the community. This role is ideal for individuals who enjoy autonomy, flexible scheduling, and meaningful one-on-one patient care.
Requirements
- Active Florida RN license
- One year of nursing experience (home health preferred but not required)
- Valid driver’s license and reliable transportation
- CPR certification
- Level 2 background screening (or willingness to obtain)
- Strong clinical assessment skills
- Ability to work independently

Responsibilities
- Complete Start of Care (SOC) assessments in accordance with state and agency guidelines
- Perform Skilled Nursing Visits including wound care, medication management, patient education, and chronic disease monitoring
- Develop and update individualized care plans
- Communicate with physicians, case managers, and the interdisciplinary team
- Document accurately and timely in the EMR
- Provide high-quality, compassionate care to patients in their homes
