About The Position

Requirements

• Minimum of 2 years nursing experience
• Current Pennsylvania licensure as a Registered Professional Nurse or eligible for State Board Licensure.
• UPMC accepted national Wound and Ostomy certification preferred.
• CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
• Exemplary clinical judgment with critical thinking, analytical and problem solving abilities required.
• Ability to establish and maintain positive, caring relationships with executives, managers, physicians, and non-physician providers.
• Ability to work productively and effectively within a complex environment and handle multiple and changing priorities.
• Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
• Registered Nurse (RN)
• Act 31 Child Abuse Reporting with renewal
• Act 33 with renewal
• Act 34 with renewal
• Act 73 FBI Clearance with renewal
• Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.

Responsibilities

• Demonstrates critical thinking in identifying clinical, social, safety, psychological, and spiritual issues for patients within an episode of care.
• Maintains ongoing follow-up evaluation for appropriate ostomy and/or skin care patients with needs after discharge from the hospital to provide continuity of care.
• Incorporates national professional organizations (NPUAP and WOCN) and health system goals to improve patient quality, safety, and satisfaction.
• Creates a caring and compassionate patient-focused experience by building healing relationships with patients, families, and colleagues.
• Demonstrates passion for developing self and others by precepting students, graduate nurses, new staff, medical students, and residents.
• Supports continuous learning by sharing research and materials from professional organizations.
• Conducts in-services and formal lectures for nursing units and other members of the healthcare team.
• Contributes to the professional development of the unit-based skin care committee.
• Identifies opportunities for clinical quality and workplace improvement.
• Applies evidence-based practice.
• Participates in product evaluation for ostomy and skin care needs to ensure cost-effective quality patient care.

• Serves as a role model of professional nursing practice, acting as a leader in clinical expertise, teacher, specialized resource nurse, and/or evidence-based practice in the nursing care of patients with alterations in skin integrity, wounds, ostomies, fistulas, and/or incontinence.
• Gives appropriate feedback to the healthcare team and participates in the peer review process.
• Promotes change that enhances patient care quality within the WOCN and Nursing departments.
• Serves on and leads teams as part of daily operations.
• Demonstrates highly developed clinical assessment and analytical skills within the context of Relationship Based Care.

• Formulates goals and a plan of care that involves the patient as a partner and considers individual needs holistically.

• Communicates the plan of care to nursing staff, physicians, patients, families, and other healthcare team members.
