The PRN Case Manager works on an as needed basis and coordinates the care and service of selected patient populations across the continuum. This role involves working collaboratively with an interdisciplinary team, both internal and external to the organization, to improve patient care through effective utilization and monitoring of health care resources. The Case Manager assumes a leadership role to achieve desired clinical, financial, and resource outcomes. The position requires excellent written and verbal communication skills, well-rounded critical thinking skills, creative problem-solving abilities, and proficient organization and planning. The individual must be able to tolerate frequent interruptions and a demanding workload, possess knowledge of funding resources, services, clinical standards, and outcomes, and be capable of performing multiple tasks simultaneously.
