About The Position

The RN/LMSW Case Manager delivers comprehensive care to a set of assigned patients at a specific point in time. The role assesses patient and care-team needs and develops, implements, and evaluates an appropriate Plan of Care, making changes in response to changing patient needs. The position is onsite at Cayuga Medical Center in Ithaca, New York, is full-time and day shift, and may involve less than 25% travel.

Requirements

Graduation from an accredited nursing or Master of Social Work program.
Active New York State RN or LMSW licensure.
Minimum 2 to 3 years of experience in an acute care hospital setting or similar health care case experience.
Complete Case Management Certification or equivalent within 18 months of hire.

Responsibilities

Evaluate hospital admissions using evidence-based standards of care to ensure care is medically necessary and delivered at the most appropriate level of care.
Communicate with providers and care teams to ensure medical necessity and patient status are appropriately captured and documented.
Participate in multidisciplinary rounds as available.
Serve as discharge planner for assigned patients, assessing readmission risk, ability to self-manage, and appropriate community support systems.
Ensure patient demographics, insurance, primary care provider, and advance directives are up to date in the EMR.
Facilitate referrals to support teams and community-based organizations.
Identify and eliminate barriers to discharge.
Issue federal and state notices as indicated.
Assume other responsibilities as needed.
