About The Position

The Inpatient Rehab Case Manager RN supports patients and families throughout their care journey by coordinating services, connecting them with resources, and developing individualized care plans. The role partners with physicians, nurses, department leaders, and the interdisciplinary team to support safe transitions, clear communication, and consistent quality across the continuum of care.

The Case Manager orients patients and families to the program and rehabilitation philosophy, explains Medicare and insurance benefits, discharge criteria, and patient rights, and coordinates education to encourage participation in treatment planning, goal discussions, and family conferences. The role also completes psychosocial assessments and discharge planning evaluations, documents findings, and communicates patient needs across the care team.

This position collaborates closely with the Rehab Program Director and Facility Case Management Director to provide seamless, patient-centered care and promotes HCA Healthcare’s values of compassion, respect, and excellence in interactions with patients and families.

Requirements

• Registered Nurse (RN).
• Associate degree.

Responsibilities

• Guide patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans that reflect patients’ strengths, needs, and personal recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, document findings, and communicate needs across the care team.
• Collaborate with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to support seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence through patient and family interactions.
