About The Position
This is a travel Physical Therapist position in a Skilled Nursing Facility located in Laconia, NH. The assignment is for 13 weeks, with a start date of 10/05/2026. The role guarantees 35 hours per week, with a weekly pay range of $2,534-$2,638. The typical shift is 5x8 Days, from 08:00 to 16:30. This position is managed by Healthforce Healthcare LLC.
Requirements
- Active Physical Therapist license in New Hampshire or eligible for licensure
- Doctor of Physical Therapy (DPT) degree
- Current CPR/BLS certification or ability to obtain
- Ability to work effectively in a Skilled Nursing Facility setting
- Relevant clinical experience in physical therapy
- Eligibility to work in the United States
Nice To Haves
- Geriatrics/SNF experience preferred
Responsibilities
- Perform initial evaluations and ongoing re-assessments of residents to determine PT needs
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and function
- Lead therapeutic exercises, gait training, manual therapy, and ROM activities as appropriate
- Monitor patient progress, adjust plans of care, and document outcomes in the chart
- Collaborate with the interdisciplinary team to coordinate care and discharge planning
- Educate residents and families on home exercise programs, safety, and fall prevention
- Ensure patient safety, infection control, and adherence to regulatory standards during therapy sessions
