Requirements

• Pharm.D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy.
• Excellent written and verbal communication skills.
• Professional experience working effectively with individuals from a variety of backgrounds and perspectives.
• Current or potential licensure by the date of hire by the Iowa Board of Pharmacy Examiners.

Nice To Haves

• Experience in a hospital pharmacy setting is highly desired.
• One year of applicable pharmacy experience; time spent completing an ASHP-accredited pharmacy residency counts as applicable experience.
• Satisfactory completion of an ASHP-accredited PGY1 residency in hospital pharmacy practice is desirable.
• Completion of an ASHP-accredited PGY2 residency in pediatrics is highly desirable.
• Board certification in Pharmacotherapy or Pediatrics is highly desirable.

Responsibilities

• Prepare and dispense sterile, non-sterile, and hazardous medications, pre-packaged medications, automated dispensing cabinet restocks, emergency drug trays, and ambulance kits in accordance with orders and hospital protocols.
• Oversee pharmacy technicians, guide and direct their workflow, ensure compliance with regulations and hospital policies, and provide performance feedback.
• Ensure the accuracy and appropriateness of dispensed medications; assist with inventory control and quality control; and address medication-related issues or discrepancies.
• Maintain current knowledge and competency in USP regulations related to medications dispensed.
• Evaluate inpatient and clinic medication orders and prescriptions for accuracy and appropriateness, including indication, dose, route, interactions, organ-function adjustments, incompatibilities, adverse effects, duplicate therapies, potential misuse, and cost-effective treatment.
• Perform pharmacokinetic assessments for appropriate patients, establish therapeutic goals, and prepare and communicate formal dosing recommendations.
• Communicate with prescribers about patient medication needs, evaluate adverse drug reactions, provide drug information, and confirm or clarify documented drug allergies.
• Complete medication histories, reconcile home medications on admission, and review and reconcile medications when patients transfer between clinical services.
• Review therapeutic monitoring information and high-risk medication therapy for patients served by the assigned care units and pharmacist team.
• Complete competency exams and compliance training, and coordinate resolution of financial concerns with Department financial counselors.
• Document medication errors, interventions, pharmacokinetic recommendations, and other pertinent patient information; update allergies and patient-specific height and weight information as needed.
• Advocate for patients and positively influence therapeutic decisions affecting their medication therapy and care.
• Coordinate medication orders during assigned duty hours, review and verify orders, complete required documentation for inpatient chemotherapy patients, respond to floor order questions, and assist with order triage when able.
• Follow up on medication-system problems, including late parenteral nutrition orders, high-cost drug therapy, non-formulary drug requests, missing doses, and unclear or inappropriate orders.
• Participate in multidisciplinary initiatives and serve on intra- and interdepartmental committees as required.
• Precept pharmacy students and residents, present educational programs, conduct patient medication education classes where appropriate, and mentor and serve as a role model for pharmacists.
• Participate in appropriate research activities, including supporting Investigational Drug Service orders and prescriptions.
• Pursue learning and professional development that support current responsibilities, future roles, and institutional goals.
• Represent the University and unit leadership in using resources to meet service and productivity demands, and promote process and quality improvement.
• Exercise functional and administrative supervision over support staff and students.
