About The Position

Vanderbilt Health is the region’s largest comprehensive academic health system, dedicated to providing world-class care, education, and research. The Clinical Pharmacist in Pediatric Neurology will work directly with Neurology providers and patients, offering comprehensive pharmacy services in an outpatient setting. This role involves medication reconciliation, optimizing pharmacotherapy, patient counseling, prior authorization review, assisting with retail prescriptions, and guiding patients through outpatient pharmacy services. Collaboration with pharmacy, nursing, and medical staff, along with strong communication skills, is crucial. The position emphasizes leadership in specialized healthcare teams to ensure optimal, cost-effective drug therapy, including developing pharmacotherapeutic goals, recommending regimens, and creating monitoring plans. The role also focuses on preventing and reporting medication errors and adverse drug reactions, continuously improving processes for medication therapy safety.

Requirements

  • Bachelor's degree (Required)
  • LIC-Pharmacist License - Tennessee
  • 1 year of relevant work experience

Nice To Haves

  • Completion of ASHP accredited PGY-1 residency program with pediatric or neurology experience
  • 2 years of pediatric experience

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
