About The Position

Planned Parenthood of Southern New England (PPSNE) is seeking a Hybrid Float Clinician to provide reproductive health care across various health centers and through telehealth. This role involves direct patient care, telehealth sessions, asynchronous e-visits, EHR Inbasket management, and support for the Care Coordination Team. The clinician will float to different health centers, working under the supervision of the Center Manager and in collaboration with the health center care team. When working with the Care Coordination Team, under the supervision of the CCT Manager, the clinician will manage follow-up of abnormal results, sign off on lab reports, and provide clinical support. Communication with patients regarding complex care plans is also a key responsibility. PPSNE is committed to providing comprehensive reproductive health services, education, and social justice initiatives.

Requirements

  • Graduation from a recognized Nurse Practitioner, Physician Assistant or Nurse Midwife training program or Medical School.
  • Appropriate professional certification (ANCC, NCC, ACNM, NCCPA or MD).
  • Active licensure in good standing (State Medical License for CT and/or RI, Controlled Substance Registration, DEA).
  • Basic Life Support certification.
  • Experience in sexual and reproductive healthcare.
  • Demonstrated leadership skills – ability to lead teams and desire to work as part of an interdependent team.
  • Sound decision making- ability to both collaboratively and independently make decisions.
  • Demonstrated project management skills, including excellent follow through for multiple projects at multiple locations, and ability to work independently as needed.
  • Comprehensive understanding of the various follow-up plans outlined by affiliate guidelines.
  • Excellent customer service skills; ability to provide quality services to meet the individual needs of patients.
  • Superb attention to detail and ability to follow processes and procedures from beginning to end within established timelines.
  • Ability to utilize data to inform the decision-making process.
  • Desire to work as part of an interdisciplinary team to meet the needs of patients.
  • Strong supporter of PPSNE mission and all reproductive health services offered – alignment with and ability to lead through the agency’s core values and diversity philosophy.
  • Good interpersonal skills communicating with patients and co-workers.
  • Ability to operate the required equipment.
  • Ability to handle discreet, confidential information.
  • Flexibility to pivot scheduled job role including covering unplanned health center absences.
  • Maintains active licenses and certifications as required to fulfill the role, including but not limited to state medical license, DEA license, and BLS Certification.
  • Ability to travel to different sites at the affiliate, as assigned, to provide clinical coverage.
  • Flexible in changing schedule to meet patient access needs at assigned centers.

Responsibilities

  • Provide in-clinic patient care within a designated region.
  • Cover telehealth sessions, working from a health center or administrative building.
  • Cover asynchronous e-visits.
  • Support health center teams by managing EHR Inbaskets and follow-up.
  • Assist as needed in the Care Coordination Team.
  • Manage follow-up of abnormal results, sign off on lab reports, and provide clinical and clinician support to the CCT.
  • Communicate with patients regarding more complex care plans, such as beta quant HCG follow-up and urgent follow-up.
  • Perform physical exams, order and interpret appropriate laboratory testing, and recognize deviations from normal.
  • Perform and interpret limited pelvic ultrasound.
  • Implement individualized regimens of care in compliance with PPSNE medical policies and procedures, including patient education and referral of services.
  • Initiate appropriate tracking of abnormal results and referrals.
  • Complete documentation of patient care in the medical record in a timely and complete manner.
  • Maintain appropriate billing and coding practices.
  • Provide telephone triage for incoming clinical calls received by the call center, as needed.
  • Complete follow-up on laboratory and imaging results per PPFA Medical Standards and Guidelines (MS&G).
  • Provide medication abortion and participate in the evaluation and management of patients receiving procedural abortion services.
  • Maintain clinical quality assurance standards as set forth by PPSNE.
  • Recognize the boundaries of one's own knowledge and utilize appropriate channels for support.
  • Participate in providing all medical services offered by PPSNE after appropriate training and privileging.
  • Demonstrate proficiency in follow-up policies, procedures, and practices.
  • Sign off on lab/imaging reports and create follow-up plans for abnormal lab or imaging results, in adherence with affiliate protocols.
  • Document clearly and comprehensively so that the CCT and health center staff can easily carry out notifications and reminders for each follow-up plan.
  • Support the agency’s mission and all the sexual and reproductive health services offered.
  • Complete all responsibilities according to established protocols, policies, and standard practices in the areas of customer service, quality assurance, and regulatory compliance programs such as HIPAA, OSHA, and CLIA.
  • Maintain active licenses and certifications as required to fulfill the role.
  • Complete reappointment process with Credentialing every three years.
  • Support quality improvement activities such as performing chart audits.
  • Partner with the Center Manager in supervision of clinical care and training of Clinic Assistants and volunteers.
  • Demonstrate flexibility, leadership, and teamwork.
  • Participate in the training of other clinicians and precept medical learners in consultation with the Chief Medical Officer, Medical Director, and Director of Clinical Services.
  • Support a culture of feedback; being open and willing to give and receive feedback.
  • Provide a rich exchange of information and ideas between sites and departments.
  • Work collaboratively with volunteers to achieve the agency’s mission.
  • Meet goals and key performance indicators for creating patient access to care within PPSNE’s care team models.
  • Participate effectively in health center efforts to achieve patient access and revenue cycle goals.
  • Deliver patient care consistent with client expectations and quality initiatives.
  • Adhere to affiliate policies and procedures on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints.
  • Contribute to quality improvement goals and activities.
  • Ensure that interactions incorporate PPSNE values of integrity, partnership, client focus, communication, and quality.
  • Support provision of services that are culturally and linguistically competent for our diverse patient population.
  • Take the initiative to develop own awareness and knowledge of the diverse backgrounds of the communities we serve.
