About The Position

Lakeland General Surgery and Weight Loss Center, Niles, is looking for a full-time APP to join their growing team! This position consists of 50% hospital-based rounding and 50% clinic time. The clinic is open Monday - Friday with flexibility. Hospital rounding starts at 0700, and clinic days start at 0800. Enjoy a great collaborative and collegial relationship with the physicians and your colleagues with a team approach! Epic EHR is utilized. In collaboration with or under the supervision of the Medical Director, Supervising Physician and Practice Manager/Director, the APP provides health care services and consultation for patients and families, focusing on wellness and health promotion. The APP serves as a leader, educator, and researcher for patients, their families, and the community. Responsibilities include performing complete screenings and examinations, as well as assessments, devising a plan of care to determine a baseline, reflect the nature of the patient visit, diagnose and promote good health. The APP will perform, order, and interpret appropriate laboratory tests and notify patients of results as needed. They will assess information and/or symptoms of a patient by phone or other technology in order to prescribe treatment or refer to appropriate resources. The APP will consult and instruct patients to provide support, facilitate recovery and foster patient self-care at home, promoting health through continued follow-up care. They will arrange for or refer patients to needed services that cannot be provided at the clinic and assure that adequate patient health records are maintained and transferred as required when patients are referred. The APP will participate in the development, execution and annual review of the written policies governing the services the clinic furnishes as appropriate. Other duties as needed and appropriate to the scope of practice will be performed. The APP is committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adherence to the organization’s published values and participation in clinic quality improvement activities are expected.

Requirements

  • Master's Degree MSN-Nurse Practitioner.
  • 1 year of relevant experience current, relevant clinical experience as nurse practitioner
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • LIC-Nurse Practitioner (NP) - STATE_MI State of Michigan Upon Hire Required, or Master's Degree in Physician Assistant Studies Required
  • 1 year of relevant experience current, relevant clinical experience as physician assistant
  • LIC_Physician Assistant (PA) - STATE_MI State of Michigan Upon Hire Required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association. 90 Days Required
