About The Position

To provide case management services, counseling, intervention and implementation of Individuals’ plans to assist in developing, maintaining, increasing and maximizing independent functioning in the skill areas identified in the Individual Service Plan (ISP).

Requirements

  • Associate's degree from an accredited college or university; and one to two years related experience and/or training; or equivalent combination of education and experience.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, Individuals, customers, and the general public.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of instructions dealing with several variables.
  • Must have reliable transportation, possess and maintain a valid California Driver's License and auto insurance.
  • Proficient in Microsoft Suite.

Nice To Haves

  • Standard First Aid and CPR certification, preferred. Certification will be provided within the first 90 days of employment at the expense of the Agency.

Responsibilities

  • Ensures that all Individuals have appropriate goals and objectives.
  • Ensures all supporting paperwork is completed and updated.
  • Ensuring all data related to production is accurately recorded.
  • Ensures quality relationships are maintained between referral agencies and The Arc.
  • Ensure the safety and well-being of each individual.
  • Be a role model by following Basic Principles and Shift Happens philosophies.
  • Schedule, conduct and document all meetings pertaining to Individuals.
  • Train all line staff on proper documentation.
  • Review weekly calendars to ensure enriching and meaningful activities are taking place.
  • Completes tours to potential individuals interested in The Arc’s Services and Supports.
  • Ensures Purchase Order of Services and Medical Forms are received and correct before an individual begins services.
  • Coordinates all services for assigned caseloads and requests any additional or cancellation of Services from the funding source.
  • Provides consultation as appropriate to assist program staff and Individuals in achieving goals and objectives.
  • Creates and implements ISP's.
  • Completes 30-day, Special Staffings, semi-annual and annual paperwork as required by each program.
  • Provides individual/group information in the areas of community engagement, volunteer opportunities, personal and or social adjustment.
  • Job coaching in the community may be required.
  • May need to attend medical appointments or engage individuals with community resources as needed.
  • Monitors and Completes Community Integrated Employment (CIE) incentive requests for the funding source.
  • Provides feedback to the supervisor regarding effectiveness of programming and proposing modifications when appropriate.
  • Writes and follows through on implementation of meaningful and attainable objectives to ensure Individuals’ growth.
  • Assures that all forms required by referral sources are completed.
  • Monitors billing throughout the month to ensure that Services are being completed correctly and ensures that an individual does not exceed their allotted hours without approval from the referral source.
  • Provides feedback and 1:1 training to Staff that may not understand how to properly case note, complete attendance, and/or complete billing.
  • Consults as a member of an interdisciplinary team.
  • Ensures all Individual’s records are accurate and up to date.
  • Completes site visits/Spends time in classroom or in the community and interacts with Individuals and staff on a regular basis.
  • Ensures Arc standards and regulatory standards are met.
  • Monitors the quality and content of the direct line staff's documentation.
  • Monitors training of the Individuals, making recommendations to the supervisor as needed.
  • Maintains professional growth by attending in-service training sessions and by reading related literature.
  • Maintains, updates, and produces reports in electronic Individual record.
  • Reports issues to Program Manager and/or Director of Programs.
  • May be required to perform the duties of a direct care staff in order to meet the needs of the Individual and/or program.
  • Passes medication and follows documentation protocol as written in the Medication Monitory policy.
  • Assists in providing care for Individuals with restricted health care conditions.
  • Follows protocol as listed in the Restricted Health condition policy.
  • Completes and/or communicates documentation requirements (i.e., Exit summary, Access to Services, incident reporting, etc.).
  • Performs other duties as assigned to ensure efficient operation of the program and/or agency.
  • May supervise developmentally disabled Individuals. Responsibilities may include training, planning, assigning, and directing work; addressing complaints and resolving problems.
