Requirements

  • One year of RN experience required
  • Licensed Registered Nurse in State of Indiana
  • Current driver's license
  • Must have reliable transportation and insurance

Nice To Haves

  • One year Hospice experience preferred

Responsibilities

  • Report to Clinical Coordinator and/or Intake Nurse each shift to learn of assigned visits/tasks for that day.
  • Common assignments may include: Hospice evals or assessments, Urgent or change of condition visits, General visit coverage for a different RNCM when on PTO or when staffing demand is present, Tuck-in visits, GIP visits, Other misc. PRN visits.
  • Appropriate follow-up / coordination of care with the involved RNCM or other involved team members after completing a visit.
