Hospice Nurse Jobs

The Hospice Registered Nurse provides compassionate, patient-centered care by assessing patient needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team. The nurse performs initial assessments for new admissions; completes, coordinates, and revises plans of care; maintains continuity of care through collaboration with appropriate staff; and communicates patient progress to the clinical manager, physician, and care team. The role also includes maintaining accurate and timely progress records, recording pain and symptom management changes and outcomes, educating families, making appropriate hospice-discipline referrals, and providing on-call services as assigned.
