RN Transitional Care Manager, Hospice

About The Position

Jefferson Health at Home by BAYADA Hospice is seeking an RN Transitional Care Manager. This role focuses on building and strengthening relationships with referral sources, particularly hospital and facility contacts, to educate them about BAYADA services and the benefits of hospice care. The manager will use clinical knowledge to inform patients and hospital staff about hospice eligibility and expectations, manage referral intake, and coordinate safe discharges to the next care setting or initiate GIP (General Inpatient) level of Care.

Requirements

  • Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability.
  • Current license in good standing as a Registered Nurse in the state of practice.
  • Graduate of an accredited and approved program as indicated by school transcript or diploma.
  • Two (2) year experience in health care.
  • Completed Continuing Education Requirements (CEUs) as required by regulation.
  • Demonstrated record of building and maintaining referral relationships.
  • Demonstrated knowledge of the Medicare COPs and other program requirements, as applicable.
  • Ability to provide excellent customer service.
  • Demonstrated ability to work well in a team environment and matrix organization.
  • Working knowledge of business office technologies: PC/laptop w Microsoft Office applications, smartphone, website/internet, email.

Nice To Haves

  • Experience in one of these settings: Hospice RN, Clinical Liaison, RN Discharge Planner, RN Educator.
  • Previous discharge planning experience.

Responsibilities

  • Demonstrate and communicate the core values of BAYADA and The BAYADA Way.
  • Develop relationships, in accordance with The BAYADA Way, with referral sources, physicians and other healthcare professionals.
  • Sell BAYADA services by clearly articulating the features and benefits of BAYADA to generate growth in the volume of referrals generated from assigned accounts.
  • Demonstrate mastery of clinical and regulatory hospice care eligibility criteria.
  • Review and compile all documentation necessary, in coordination with referral sources, to transition care from an acute care setting/ sub-acute care setting or initiate care as ordered by a physician to BAYADA Hospice.
  • Coordinate with the service office to set up the services ordered by the physician.
  • Completes the evaluation on hospital referred patients for community and for inpatient hospice admissions.
  • Provides accurate and timely information regarding hospice team and hospice staff.
  • Participate in office’s quarterly business planning and market analysis to achieve growth goals.
  • Serves as patient advocate in evaluating/developing plan of care for GIP patients and obtaining informed Consent for hospice.
  • Providing daily GIP visits and documenting to GIP eligibility.
  • Collaboration with Business Development Manager and/or Director on key and shared accounts.
  • Attend and participate in weekly team growth meetings.
  • Cross-train office field staff to cover absences, as needed.
  • Participate in community and sales activities, as needed.
