About The Position

Armor Health is hiring a Discharge Planner/Licensed Practical Nurse to join its team at Franklin County Jail in Columbus, Ohio. The position supports discharge and pre-release health planning, coordinating referrals and care for patients with chronic, complex, or specialized needs. The role works with medical providers, behavioral health professionals, and multidisciplinary programs to support continuity of care after release from custody. The schedule is described as 3x12s, night, 7 a.m.–7 p.m.

Requirements

At least two years of experience, preferably in correctional health or a criminal justice environment, as a case manager, discharge planner, nurse performing similar referral, case management, or care coordination tasks, or mental health-related service coordinator.
A bachelor’s degree in social sciences, human services, or a healthcare-related field, or a nursing degree.
Excellent written and verbal communication skills.
Excellent computer skills.
Familiarity with health records, medical terminology, and health information services.

Nice To Haves

Experience in correctional health or a criminal justice environment is preferred.

Responsibilities

Monitor chronic care logs and caseloads as needed to supplement referrals from staff.
Develop discharge-planning referral criteria for intake medical screeners.
Consult routinely with medical providers about the current caseload to identify patients who meet criteria for pre-release health planning.
Consult with Armor behavioral health providers, including psychiatrists and licensed mental health professionals, about patients with complex needs requiring additional coordination beyond individual treatment or behavioral health referral plans upon release.
Participate in multidisciplinary planning for specialized transitional programs serving seriously or chronically ill patients, such as community-based mental health teams and HIV Jail Linkage programs.
