About The Position

Requirements

• Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school.
• CPR Certification required.
• Board Certification or Board Eligibility.
• Connecticut Licensure.
• Must have at least 5 years of clinical experience in outpatient internal medicine or family practice.

Nice To Haves

• Prior experience working in addiction medicine preferred.
• Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.

Responsibilities

• Conducts comprehensive medical evaluations, education, and treatment.
• Maintains patient confidentiality in compliance with agency, state, and HIPAA laws.
• Assesses medical and psycho-social histories, performs physical exams, identifies problems, formulates treatment plans, provides education and referrals, and evaluates intervention effectiveness.
• Orders and interprets laboratory tests, reports communicable diseases to the Health Department as per protocol.
• Prescribes appropriate medication.
• Implements standardized evaluations or assessments with a multidisciplinary team.
• Ensures complete, timely, and accurate documentation.
• Understands medical emergency procedures and the location of emergency supplies.
• Participates in continuing medical education to enhance professionalism and quality of care.
• Provides on-call coverage as scheduled.
• Maintains good working relationships with community agencies.
• Attends and participates in team conferences, huddles, and required trainings.
