About The Position

Requirements

• Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education, with a BS or PharmD degree.
• Licensed as a Registered Pharmacist by the Board of Pharmacy of the state in which the employee will work.
• Pharmacists in the SMPHM Union must be licensed in both Minnesota and Wisconsin within one year of hire, regardless of primary worksite, if they are regular full-time pharmacists or part-time pharmacists working at least 0.4 FTE.
• If not licensed as a Registered Pharmacist upon hire, the candidate must have a Pharmacy Intern License and/or be eligible to work as an intern and become a Registered Pharmacist within 90 days of hire.
• Maintain continuing education and renew licensure as required by the state's Board of Pharmacy.

Nice To Haves

• ASHP-accredited hospital residency or three or more years of hospital pharmacy experience.
• Completed or currently completing a PGY2 ASHP-accredited residency in Infectious Disease or Critical Care.
• Infectious Disease or Critical Care Pharmacy experience.
• Board of Pharmacy Specialties (BPS) board certification in Infectious Disease or Critical Care.

Responsibilities

• Interpret, verify, dispense, and monitor medications ordered by prescribers.
• Evaluate and disseminate information about medications and their use to patients, families, and medical staff.
• Provide quality pharmaceutical care.
• Perform medication monitoring, distribution, and acquisition duties as required.
• Perform medication reconciliation, anticoagulation dosing and monitoring, pharmacokinetic monitoring, and renal dose adjustments as needed.
• Provide medication-related consultative services to Essentia Health staff.
• Assist with daily departmental activities, including training new staff and precepting pharmacy students, interns, and residents.
• Assist with billing, record keeping, monthly audits, and other duties assigned by the department leader.
