About The Position

This role involves providing case management services within an ECF (Extended Care Facility) setting. The RN Case Manager will utilize their clinical background and critical thinking skills to assess patient needs, develop care plans, and collaborate with facility staff, patients, and families. A strong understanding of hospice philosophy, pain and symptom management, and adult learning principles is essential. The position requires the ability to work independently and as part of an interdisciplinary team, with a focus on effective communication and organizational skills.

Requirements

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as an RN through the Ohio Board of Nursing.
  • Must be able to obtain a license in the state of Michigan.
  • Minimum 2 years of medical-surgical nursing experience.
  • Working experience charting in an EMR system.
  • Strong clinical background with knowledge of disease processes and pathways.
  • Clinical expertise related to critical thinking, assessment, problem-solving and technical skills.
  • Ability to multitask.
  • Ability to work independently and within an interdisciplinary team.
  • Must be flexible and adaptable to change.
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
  • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
  • Meets state requirements for maintaining professional license.
  • Must be CPR certified according to the American Heart Association CPR standards.

Nice To Haves

  • BSN preferred.
  • Previous experience in the nursing home environment and/or hospice is preferred.
  • Netsmart system preferred.
  • Knowledge of Medicare hospice regulations and Joint Commission regulations preferred.

Responsibilities

  • Seek, form, and foster an effective, collaborative caregiving partnership with nursing facility staff.
  • Demonstrate awareness and sensitivity to the unique mission and cultural climate of each facility environment.
  • Communicate effectively with patients, families, and team members regarding care issues.
  • Work independently and within an interdisciplinary team.
  • Manage organizational, time management, and prioritization tasks effectively.
  • Maintain professional license and CPR certification.
  • Chart in an EMR system.
