About The Position

The Resource Coordinator (Care Manager) participates in outreach and enrollment and provides individuals, family members, and caregivers with guidance about available services. As part of an interdisciplinary team, the coordinator provides service coordination and manages the medical, social, and behavioral health needs of enrolled members. The role oversees and facilitates access to services that help individuals and families stay healthy, prevent hospitalizations, and maintain stability. Home visits are required in the Queens or Long Island area.

Requirements

Bachelor’s degree in Human Services or a related field.
Two years of experience working in Human Services, Psychology, or a related field, including coursework in principles of social work, child development, counseling, or psychology.
Ability to plan and carry out assignments independently.
Ability to prioritize, meet timelines, and multitask.
Ability to conduct required home visits.

Responsibilities

Complete intake, enrollment, and ongoing assessments.
Facilitate interdisciplinary collaboration among providers, enrollees, families, caregivers, and available supports.
Ensure families know about support services and entitlements; recommend and refer them to relevant community resources.
Advocate for individuals and families, as needed, by serving as a liaison between them and other programs.
Conduct office, home, and community visits to assess family needs and service delivery.
Conduct outreach to program candidates in community client populations.
Coordinate services for people with special needs and support families of children with medical and mental disabilities.
Monitor and maintain compliance with Federal, State, and Local regulations.
Monitor compliance using data management systems.
