About The Position
Join our team as a Case Manager! At NeuLife Rehabilitation of Michigan, we're more than just a workplace; we're a community committed to compassion, quality care, and supporting both our persons served and our team. As a Case Manager, you will coordinate care and rehabilitation services for individuals with traumatic brain injuries and spinal cord injuries. You will collaborate with the interdisciplinary team, clients, families, guardians, payers, and external case managers to develop individualized care plans, monitor progress, and support transitions of care. This role requires timely communication, accurate documentation, and advocacy for each individual's needs and rehabilitation goals.
Requirements
- At least three years of case management experience, preferably in rehabilitation or a related healthcare setting
- Bachelor's degree in a related field preferred
- Experience with brain injuries or neurological conditions and CCM or ACM certification preferred
Responsibilities
- Assess needs and coordinate individualized care plans with the interdisciplinary team
- Monitor progress and communicate updates to clients, families, guardians, and providers
- Coordinate care conferences, utilization reviews, and payer authorizations
- Support discharge planning, referrals, and transitions of care
- Maintain accurate documentation and track quality measures
