About The Position

Requirements

• Valid HHA or CNA certification, or willingness to complete HHA certification course
• Current CPR and First Aid certification
• Passing background check
• TB/PPD test required prior to employment
• Valid driver's license, reliable vehicle, and current vehicle insurance
• High school diploma or GED
• Legal authorization to work in the United States
• Physical ability to perform job duties including lifting, assisting with mobility, and walking for extended periods
• Ability to document care and communicate via phone and electronic systems

Responsibilities

• Provide person-centered care as outlined in individual service plans, including but not limited to assistance with mobility and transfers, bathing, dressing and grooming, and meal preparation
• Work independently in clients' homes following established care protocols
• Document care activities in the patient care portal
• Maintain reliable weekly availability and willingness to travel throughout service areas
